Hillcroft’s Home & Community Services provide support to people with disabilities in a variety of settings. Supports include respite care, 24-hour supported and group living sites and scheduled drop-in services. Hillcroft’s Home & Community Services provides a well-trained group of professionals who offer children and adults with disabilities assistance and training to increase independent community living skills.
Supported Living Services
Supported Living Services provide up to 24-hours a day of services and supports which are designed to ensure the health, safety and welfare of the participant, and assist in the acquisition, improvement, and retention of skills necessary to support participants to live successfully in their own homes. Creativity Unlimited
Creativity Unlimited (CU) programming is based upon the specific needs and interests of the participants. Instruction areas include life skill development, driver’s education, computer usage, educational developmental, social relationships, community access activities, mobility instruction, recreation and self-advocacy. Group Home Services
Group Homes provide 24-hours a day of services and supports to assist and train each participant in developing daily living skills and to ensure the health, safety and welfare of the participants. Learn More>>
Wellness Coordination
Wellness Coordination is the development and routine monitoring by Registered and Licensed Nurses of the clients Wellness Coordination Plans and medical services required to manage their health care needs. Learn More>>
Respite Services
Respite Services are designed to provide short term support to individuals who are unable to care for themselves enabling relief to the primary caregiver(s). Respite Services are typically provided in the family’s home. Learn More>>
